A HIPAA-compliant form builder is a tool for collecting protected health information (PHI) through online forms, backed by a signed Business Associate Agreement (BAA), encryption, access controls, and audit logging. The leading options in 2026 fall into four groups:

  • General form builders with a HIPAA tier: Jotform, Formstack, Cognito Forms, Zoho Forms, and Typeform Enterprise
  • Forms inside your productivity suite: Google Forms and Microsoft Forms, for organizations with a correctly configured Workspace or Microsoft 365 tenant

Who This Comparison Is For

This page is for clinics, practices, digital health companies, and healthcare operations teams choosing a tool to collect patient information online, including:

If your forms never touch PHI (event sign-ups, newsletter subscriptions, staff feedback), you don’t need a HIPAA-eligible tool, and you’ll pay considerably more for one.


Why HIPAA Form Builders Are Hard to Compare

Most form builders offer HIPAA compliance on only one or two of their plans, and the BAA covers only the form tool itself. So the headline price and the “HIPAA-compliant” label rarely tell you what you’ll actually pay, or what you’ll still be responsible for.

Four things make like-for-like comparison difficult:

  1. HIPAA is plan-gated. Nearly every vendor restricts BAAs to a higher tier. The price that matters is the cheapest plan the vendor will sign a BAA on, not the entry price on the pricing page. Do not assume a free plan includes a BAA; confirm eligibility before collecting PHI.
  2. The BAA covers the tool, not your whole workflow. Email notifications containing PHI are one of the most common leaks. A form tool’s BAA doesn’t cover what your downstream integrations and notifications do with the data.
  3. Some tools are only compliant if you configure them correctly. Google Forms, Microsoft Forms, and self-hosted WordPress plugins can all carry PHI, but only when your tenant or hosting is set up for it. The responsibility for that setup sits with you.
  4. The form is the front door, not the whole house. A submitted intake form usually has to become an appointment, a message thread, a clinical record, or a consultation. How far a tool carries that data varies more than any feature list suggests.

Category 1: General Form Builders with a HIPAA Tier

Jotform

Best for: small practices that want HIPAA forms without a sales conversation.

Jotform is a widely used general-purpose form builder, and HIPAA features are self-serve. HIPAA compliance is available only on the Gold and Enterprise plans; the free Starter, Bronze, and Silver plans don’t include a BAA. Gold costs $99 a month billed yearly, or $129 a month on monthly billing.

Jotform has also moved into conversational intake. Its AI Agents can collect patient information through a chat interface, and on HIPAA-enabled accounts they use Google’s Gemini models through Vertex AI, which is covered by a BAA.

Consider before choosing Jotform: it is a form and agent platform, not a care-delivery platform. Once intake is complete, messaging, virtual visits, and follow-up happen in other tools, each with its own compliance review. Also check how you configure notifications: Jotform’s BAA covers its own storage and processing, not what your downstream tools do with the data.

 

Formstack

Best for: organizations that need forms, document generation, and e-signature under one vendor, especially Salesforce users.

Formstack builds for regulated industries. Its Suite combines Forms, Documents, and Sign, so a patient intake form can generate a consent document and route it for signature in one platform. Pricing starts at $83 a month billed yearly ($99 monthly) for Forms, and $250 billed yearly ($299 monthly) for the Suite.

Consider before choosing Formstack: HIPAA support is listed as an add-on to the Forms and Suite plans. The published starting prices therefore do not represent the full HIPAA-enabled cost. Confirm the add-on price, BAA terms, and included products before budgeting.

 

Cognito Forms

Best for: clinical intake that needs repeating sections and scoring.

Cognito Forms handles repeating sections (one block per medication, symptom, or family member) and has built-in calculations for scoring instruments. HIPAA is available only on its Enterprise plan: $129 a month billed yearly, or $174 monthly.

Consider before choosing Cognito Forms: a small practice pays Enterprise pricing for compliance. As with other general form builders, intake data leaves the platform for scheduling, messaging, and consultations.

 

Zoho Forms

Best for: organizations already running on Zoho.

Zoho Forms offers HIPAA compliance on its Premium plan at $90 a month billed annually. Among the self-serve general form builders in this comparison, that’s the lowest-priced HIPAA tier. The plans below Premium don’t include HIPAA.

Consider before choosing Zoho Forms: the value depends heavily on already using the Zoho suite, where submissions route natively into Zoho CRM.

 

Typeform

Best for: long patient questionnaires where completion rates matter.

Typeform’s one-question-at-a-time format helps with long questionnaires, where a wall of clinical questions leads to abandonment. Typeform offers a BAA on its Enterprise and Growth Custom plans, both custom-quoted. Standard self-serve plans do not include a BAA.

Consider before choosing Typeform: its one-question-at-a-time format is not the same as AI-powered conversational intake. Confirm separately whether any AI features you intend to use are covered by your BAA. 


Category 2: Forms Inside Your Productivity Suite

Google Forms

Best for: internal, low-complexity forms in organizations already using Google Workspace.

Google Forms can carry PHI, but only inside a paid Workspace account where an administrator has accepted Google’s BAA and enabled the right controls. Consumer Google accounts are never covered.

Consider before choosing Google Forms: it wasn’t designed for PHI, and several common configurations leak data, including shared response sheets, email notifications, and add-ons. Design control is minimal, and conditional logic is limited to skipping between sections.

 

Microsoft Forms

Best for: internal clinical surveys and staff forms in Microsoft 365 organizations.

Microsoft’s BAA extends to Forms when your tenant is configured for it, at no additional cost. This may avoid a separate form-tool subscription, but configuration and compliance review are still required. Free personal Microsoft accounts aren’t covered.

Consider before choosing Microsoft Forms: it offers basic form capability with little design control, and it’s better suited to internal use than to patient-facing intake.


Category 3: Healthcare Intake Platforms

Consider before choosing IntakeQ: it’s designed as an off-the-shelf tool for individual practices. Organizations that need their own branded platform, AI-driven intake workflows, or control over where data is hosted will find less room to configure.


Category 4: Forms Connected to Care Delivery

QuickBlox

Not HIPAA-eligible: some popular form builders don’t sign BAAs on any plan, including Paperform. They’re fine for forms that never touch PHI, but never for patient information.

  2. Ask what the BAA covers. Storage and processing are standard. Check notifications, integrations, file uploads, and any AI features separately.
  3. Map where the data goes next. Identify every service that handles PHI after submission, review its security controls, and confirm any required BAA coverage.
  4. Check your configuration responsibilities. Suite-bundled and self-hosted tools shift setup work to your team.
  5. Test the patient experience on mobile. Completion rates on long forms vary widely.
  6. Confirm audit logging and access controls, in line with HIPAA technical safeguards.
  7. Decide whether you need forms or intake. A form collects data. An intake workflow also acts on it.
